Management of HTN case
Patients এর Age যদি (35-65) এর মধ্যে হয়।
First Visit এ আমরা কখনোই Antihypertensive দিবনা যদি না Patient এর Moderate Hypertension হয়।
life style modification করতে বলবো।
এখন একটা পেশেন্ট আসলো Isolated systolic hypertension নিয়ে Like
BP 160/85
চিন্তা করবো
White coat HTN
➤Rx
B-Blocker for 15 days then follow up BP
একটা পেশেন্ট আসলো Moderate hypertension নিয়ে Like
BP 160/100
4 টা test করতে দিব
1)ECG to see the complications like LVH
2)Urine Strip test ➤ for Sugar & protein
3)USG to see the corticomedullay differentiation
4)RBS
Interpretation and Management
1)ECG তে LVH পেলে
➤Mx
ARB ⇨Losartan 25mg once daily
2) Urine strip এ
Sugar পেলাম তার মানে Sugar >10mmol/L
Protein 3+++ পেলাম মানে Hypertensive or Diabetic Nephropathy developed করে গেছে।

Losartan 25mg once daily
3) RBS এ Sugar
>11.1 পেলাম চিন্তা করবো Metabolic Syndrome
Further আমরা Hb1c করতে দিব।
If Hb1c (7-9%) হয় Double antidiabetic শুরু করবো।

Glicazide+Metformin
একটা পেশেন্ট আসলো
BP 165/100 নিয়ে, Investigation করে
(LVH+Proteinuria(3+)+Poor Corticomedullary differentiation)
পেলাম।

Double Antihypertensive
⇨Amlodipine+Olmesartan
এখন একটা পেশেন্ট আসলো
BP 165/100 নিয়ে, Investigation করে
(LVH+Proteinuria(3+) +Poor Corticomedullary differentiation)
